Material Testing Request

Before You Submit a Sample

Do not ship test materials until your request has been reviewed and a test number has been issued by Cyclonaire.

Cyclonaire requires all test materials to be reviewed and authorized before shipment. Please complete the Test Laboratory Policy and Procedures form, provide the required Safety Data Sheet documentation, and work with your Cyclonaire contact to obtain an assigned test number. Materials received without the required paperwork, authorization, and test number cannot be accepted.

1. Download and complete the Test Laboratory Policy and Procedures form.
2. Contact your Cyclonaire representative or Cyclonaire Test Lab team to discuss the material, test objectives, and required sample quantity.
3. Email the completed form and an SDS for each material to your Cyclonaire contact for review.
4. Wait until Cyclonaire issues a test number before shipping any material.
5. Clearly mark the assigned test number on the outside of each package and include the required SDS and test documentation with the shipment.
6. Ship the authorized sample prepaid to:

7. Provide shipment tracking information to your Cyclonaire contact.
8. After testing, remaining material may be returned freight collect or disposed of according to the arrangements approved during the request process.

Sample Submission Disclaimer
For safety reasons, samples submitted without the required documentation, authorization, or shipping procedures may be treated as potentially hazardous and returned to the sender at the sender’s expense.

Material Disposal Notice
Materials identified as hazardous on the Safety Data Sheet are not eligible for landfill disposal by Cyclonaire. Non-hazardous materials may be disposed of by Cyclonaire for a fee of $150 for up to one ton, plus $150 for each additional ton.
